How to Refer/ Referral Form

We aim to make referrals as easy as possible for our referring practices and their clients.

Please use the online form below to submit your referral. Alternatively, you can download the following PDF documents which can be printed, filled in, and posted to us.

Please ensure all information is completed, this includes, full clinical history, including laboratory results (both normal and abnormal) and radiographs when completing the form.

Once we have received the referral, we can either make direct contact with your client to arrange the appointment or wait for the client to contact us arrange the appointment.

If you wish to discuss a pet referral before you do refer, please feel free to give us a call or send us an email at [email protected]

Emergencies

If you are making an emergency or same day referral, please complete the information on this page and then contact us on 015395 67241 to alert the relevant Specialist.
